Mast Cell Activation Syndrome and Endometriosis: Understanding the Connection

Mast cells are immune cells known for their roles in allergic reactions and inflammation. Studies reveal an increased number and activation of mast cells within endometriotic lesions compared to normal uterine tissue. These activated mast cells release inflammatory mediators that can promote pain, fibrosis, and lesion progression in endometriosis.
One key insight is the role of estrogen in this interplay. Elevated estrogen levels stimulate mast cell activation and recruitment to the endometriosis lesions. Estrogen promotes the release of substances like nerve growth factor from mast cells, sensitizing nerve cells and contributing to the debilitating pelvic pain commonly experienced by endometriosis sufferers.
Endometriotic lesions create a microenvironment rich in factors such as stem cell factor (SCF) that drives mast cell expansion and survival. This environment enhances mast cell differentiation and pro-inflammatory activity, further fueling chronic inflammation and lesion growth. Experimental mouse models treated with estrogen show increased mast cell presence in endometriotic lesions, confirming estrogen’s role in mast cell-mediated inflammation.
Furthermore, molecular pathways involving mast cells, such as activation of the NLRP3 inflammasome, have been identified as mechanisms by which mast cells exacerbate inflammation and fibrosis under estrogen stimulation.
Understanding the involvement of mast cells in endometriosis opens avenues for targeted therapies. Mast cell stabilizers and inhibitors are being researched as potential treatments to reduce pain and inflammation in endometriosis patients.
