It is the most common question we hear from fertility patients: “I just want a baby. Can’t we just skip the surgery and go straight to IVF?”

We understand the urgency. When you are struggling with infertility, time feels like your enemy. The idea of pausing for surgery and recovery feels like a delay you cannot afford. You just want to get to the destination.

But in the world of endometriosis, taking a shortcut often leads to a dead end. While IVF can bypass the fallopian tubes, it cannot bypass the Inflammation Barrier.

Here is why skipping surgery often leads to failed cycles—and why treating the disease first is actually the fastest route to a baby.

The “Toxic” Environment

Many patients view IVF as a purely mechanical process: Extract egg + Fertilize with sperm = Baby. But endometriosis is not a mechanical disease; it is an inflammatory one.

If you have active, untreated endometriosis, your pelvic cavity is in a state of chronic warfare.

  • Toxic Peritoneal Fluid: The fluid surrounding your ovaries and uterus is often filled with inflammatory cytokines (proteins) and oxidative stress markers.

  • The Result: This creates a “toxic” bath for your eggs and developing embryos. Even if you create a beautiful embryo in the lab, placing it back into an inflamed pelvis is risky. It can impair the embryo’s ability to survive and develop.

You might have a genetically perfect, PGT-A tested embryo. But for pregnancy to occur, that embryo (the seed) must implant into the uterine lining (the soil).

Endometriosis alters the immune environment of the uterus.

  • Progesterone Resistance: The lining may not mature properly to accept the embryo.

  • Immune Rejection: The heightened immune system may attack the embryo as an invader rather than welcoming it.

This is why we see so many patients with “Unexplained Recurrent Implantation Failure.” The embryo was fine; the environment was hostile.
