Interviewer: Dr. Fernandez, let’s start with you. You have deep roots here. What does it mean to bring this level of care to your home state?
Dr. Osbert Fernandez: It’s incredibly personal for me. I am a native Floridian and a proud second-generation Cuban-American. I know this community; I know the spirit of Miami. For too long, I saw patients in South Florida—from Coral Gables to West Palm Beach—struggling to find specialists who truly understood complex endometriosis. Redefining the “Modern Toolkit” for Endometriosis
Interviewer: You mentioned the “modern toolkit.” Endometriosis surgery has evolved significantly. What does that toolkit look like at ESSI Miami today?
Dr. Vidali: It has moved far beyond simple laparoscopy. The modern toolkit is about precision and technology meeting deep anatomical knowledge. First and foremost, it is about robotic-assisted excision. We use the Da Vinci systems not just because they are high-tech, but because they give us 3D visualization and the dexterity to perform microsurgery on a macro scale.
Dr. Fernandez: The most important tool is the philosophy of excision over ablation. We don’t burn the surface; we cut out the disease at the root. That is the only way to provide long-term relief.
Dr. Vidali: I would add that the toolkit also includes a “neuro-navigation” mindset. We aren’t just looking for red spots; we are identifying the hypogastric nerves, the sacral roots. We are operating in spaces that general gynecologists simply are not trained to enter.
The Critical Importance of Nerve-Sparing Surgery
Interviewer: That leads perfectly into my next question. Why is nerve-sparing surgery so critical, and how does it differentiate ESSI?
Dr. Vidali: Endometriosis is often a “neuro-active” disease. It loves to grow near nerves. If a surgeon is not trained in neuropelveology, they might remove the disease but damage the nerves that control your bladder, bowel, or sexual function. You might wake up pain-free from endometriosis but unable to empty your bladder or experiencing numbness. That is not a victory.
Dr. Fernandez: In Miami, we see so many patients with deep infiltrating endometriosis (DIE) involving the uterosacral ligaments or the rectovaginal septum. These are high-risk zones for nerves. Our approach is meticulous: we identify the nerves first, isolate them, and then remove the disease. We want our patients to reclaim their lives fully—not trade one set of symptoms for another. This focus on functional preservation is what truly sets ESSI apart in Florida.
Fertility Preservation and Adenomyosis
Interviewer: Many of your patients are also fighting for their fertility. How do you approach the patient who wants to get pregnant but has severe disease or Adenomyosis?
Dr. Vidali: This is where my background as a Reproductive Endocrinologist comes into play. We never operate in a silo. Every cut we make is weighed against the patient’s fertility goals. For Adenomyosis, the traditional answer was “hysterectomy.” That is unacceptable for a woman wanting a family.
We offer fertility-sparing options for adenomyosis, and we also focus heavily on Silent Endometriosis—patients who may not have pain but have had recurrent IVF failures. Cleaning up the inflammatory environment via excision can be the key to a successful embryo transfer.
Dr. Fernandez: And it’s about timing. Because we work as a multidisciplinary team, we coordinate with fertility specialists. We treat the ovarian endometriomas (chocolate cysts) with extreme care, stripping the cyst wall without destroying the healthy ovarian reserve. In Miami, we see patients who have been told their only option is donor eggs or surrogacy, and often, we can restore their anatomy enough to give them a fighting chance at using their own.
The Multidisciplinary Team: Bowel, Bladder, and Beyond
Interviewer: You touched on the multidisciplinary team. Why is that essential?
Dr. Fernandez: Endometriosis respects no boundaries. It invades the bowel; it invades the bladder; it can even reach the diaphragm. A gynecologist alone cannot safely treat all of these. At ESSI Miami, we don’t just “call in a general surgeon” if we find something. We plan ahead.
Dr. Vidali: Correct. If we suspect bowel involvement, we have a colorectal specialist ready. If there is ureteral involvement, we have urology support. This allows us to perform complex procedures like bowel resection or bladder shaving safely in a single surgery. Patients travel to us from all over Florida—Orlando, Tampa, Jacksonville—because they know they can get everything fixed at once, rather than needing three separate surgeries.
