Endometriosis and Rectal Pain: Symptoms, Diagnosis, and Why Surgery May Be the Best Option

Endometriosis is a complex condition that can affect multiple areas of the pelvis—including the rectum and bowel. While most people associate endometriosis with pelvic pain and painful periods, rectal pain is a lesser-known but common symptom that may indicate deep infiltrating endometriosis (DIE).

What Is Endometriosis-Related Rectal Pain?

Rectal pain in patients with endometriosis typically stems from lesions growing behind the uterus in the rectovaginal septum, on the uterosacral ligaments, or directly on the bowel wall, most commonly the rectosigmoid colon. These lesions can cause chronic inflammation, nerve irritation, and even partial obstruction of the rectum—leading to a range of gastrointestinal symptoms.

This form of deep endometriosis often goes undiagnosed for years due to the overlap with other GI disorders like IBS (Irritable Bowel Syndrome) or inflammatory bowel disease. However, the pain caused by endometriosis often follows a menstrual pattern and may worsen during periods.

Common Symptoms of Rectal Endometriosis

  • Sharp or stabbing rectal pain during menstruation

  • Pain with bowel movements (dyschezia), especially around the period

  • Constipation or straining to defecate

  • A feeling of incomplete evacuation

  • Rectal bleeding during the menstrual cycle

  • Pain with sitting, intercourse, or deep pelvic pressure

Why Surgery May Be Needed

Medical treatments such as hormonal therapy or birth control may help reduce inflammation, but they rarely eliminate deeply infiltrative rectal endometriosis. In many cases, minimally invasive excision surgery is required to fully remove endometrial lesions from the bowel wall or surrounding pelvic nerves.
