What is it? Deep gluteal syndrome is compression, inflammation, or dysfunction of the nerves of the deep gluteal space and greater sciatic region — usually several nerves at once. It is often mislabeled as “sciatica” or blamed on the spine.
The nerves involved Sciatic • pudendal • posterior femoral cutaneous • perforating cutaneous • inferior and superior gluteal nerves • proximal obturator nerve • contributions from the inferior hypogastric plexus.
Symptoms – Deep buttock pain • posterior thigh pain or radiation – Pain with sitting • burning, sharp, or electric sensations – Pelvic pain or pressure • overlapping urinary, bowel, or sexual dysfunction
Why standard care fails — It treats one nerve or one region, can’t see deep intrapelvic pathology, and misses the inflammatory or endometriotic cause.
